HVL means Half-value layer. It is the thickness of the material (here lead) at which the intensity of the radiation drops to half of its initial value. Dose rate is directly proportional to the intensity of the radiation. One 2cm thick lead shield can be considered as four 5mm lead shields put together. Each 5mm shield reduces the dose rate by half that is by a factor of 2. Therefore total reduction in dose rate is by a factor of 24 = 16. Therefore the answer is D. 16.
