The factors which should be considered while having performance failures are of poor planning. Poor planning will not guide you with the success you need for the project you're working with. If you need to find success then you should be having a plan which will help you in achieving organizational objects. If you're having poor risk management then you have to go through with failures which will demotivate you while performing any task in the organization. Communication gap is also a key factor that can impact you with your performance failure as if you're not comfortable with having communication with your other members or with your managers then it will not have a good impact on you as well as on the organization. Communication between employees should be smooth and easy to understand. Lack of coordinating is also a factor that needs to get sorted out as quickly as possible as it will not let your performance grow and it will have a bad impact on the organization's growth.
