It is actually difficult and not usual to assign moral responsibility for the computer system failures to the particular individual as one individual cannot be responsible for a major failure. There can be several issues which can lead to system failures and there are several issues which can be there which are handled by the project team as a whole. Thus when a system is being developed, tested and maintained by a project team, then how can the system failures can refer to just one particular person in general. It should be the fault of more than one actually who have missed to identify the risks and act according to handling it. Thus yes, it is difficult to assign moral responsibility for computer system failures to just one person, ideally, the entire project team is responsible for it.
