According to rules, the patient has to pay either a coinsurance amount ( 20% of the charges for surgical procedures and services) or a fixed copayment amount, whichever is less.

here,

coinsurance amount = 20% of $ 65 = 0.2 x 65 = $ 13

fixed copayment amount for this type of procedure adjusted for wages in the geographic area = $ 15

therefore,

the actual amount required to pay by the patient for the surgical procedure = copay = $ 13 ( since coinsurance amount is less than the fixed copayment amount )
