Mrs. Lai Loe, a patient with a history of advanced Alzheimer’s Disease, fell at home while trying to get out of her bed. She has no apparent injuries, and an X-ray of her back was negative for any fractures.
You, the nurse, are at the bedside to complete hourly rounds and a pain assessment on Mrs. Loe. Mrs. Loe is unable to tell you if she is in pain. She is breathing quietly and has normal rhythmic respirations. She is intermittently moaning and groaning, and groans with any sort of repositioning. Mrs. Loe is frowning and appears scared. She seems to be content while busily fidgeting with the blanket on her bed.

Is Mrs. Loe in pain? How did you determine this?
1) Older patients pain to be assessed is very challenging for the health professionals as it may be acute or chronic which can be untreated due to the overuse of medications by the older adults.
The older adults may have some of the cognitive impairments like delirium or dementia with depression which would lead to not verbalize the pain.
Besides, due to immature T cells the patient will not have temperature rise and will not show the sign of pain as increased fever.
2) cognitive impairment is a condition were a individual will lose his perception of thinking, memory and intelligence. When taking care of the patients with cognitive impairment it is very difficult to assess the pain as some time even the subjective and objective data seems to be showing no signs of pain.

3. PAINAD scale is the tool to assess the pain level in the patients who are cognitively impaired. Here the patient has complaints of moaning and groaning, fidgeting and sad grimace which comes under the score of 3.
