this is my nursing intervention for Impaired skin integrity. Every intervention need evalauation.

1. Nursing Intervention: Assessed nutritional status and referred to nutritionist.

Evaluation: Patient consumes diet high in protein, zinc and vitamins C, A and B.

2. Nursing intervention: Wound changed daily as per order and hand hygiene performed as directed.

Evaluation: Experiences healing of tissue as manifested by reduced swelling, pain and redness.
