The process of collection of medication details of the patient prior to the hospitalization is called medical reconciliation. This data includes the drug details such as drug name, drug dose, dosage schedule, dosing frequency, indication and the last dose taken. In this case, the patient is admitted for drug and alcohol abuse and the information of the medication is collected from the patient's sister, who is with the patient and takes care of her. The patient is suffering from drug abuse and cannot collect information. Moreover, the patient might have brought drugs from different pharmacies and this does not provide sufficient details. In medical records, certain drugs have been ignored by the patient and it doesn't provide accurate details of the drug consumed. Information gathered from the care-taker (patient's sister)will help to provide correct medication for the patient.
