You may choose a healthcare encounter of your own or that of a relative or friend.

The encounter may be with any healthcare provider (physician, nurse, hospital, nursing home, EMT (emergency medical technician), pharmacist, radiology provider, physical therapist, home health agency, etc.).

Requirements

provide the view of the customer,
