Medicare replacement plan or the Medicare Advantage plan is simply a replacement of the original Medicare. This means the responsibility of the claims goes to the insurer.Here in order to get certain procedures done the patient should have paid the Medicare part B because the government really pay a very minimal or no funds for this.The information the cost of services which can tskem the PPO services .In this state the insurance company needs a clear description of diagnosis and systematic approach in getting issued an approval by filling a form.This is called as propr authorisation or pre authorisation. In order to issue this the insurance company gets a complete details from the side of patient too.
