1. Patient navigator is a healthcare professional. He acts as a guide for the patient and helps the patient to navigate through the healthcare system to ensure that his needs for medical care are met. Patient navigator helps the patient to receive optimum care and treatment needed by resolving his problems and queries.

The role of nursing assistant as healthcare navigator are as follows :

  • To ensure Timely services for patient, organizing schedules and managing appointments.
  • Promoting properand clear communication between the patient, family members, and healthcare providers resulting in patient satisfaction and quality of care.
  • Managing health care records
  • Educating the patient on financial matters of care and providing patient with financial resources as needed.

PCMH model encompasses patient treatment in coordination with their primary care physician to ensure delivery of necessary care when and where they need it.
