If a client is experiencing acute pain, first a nurse should investigate the reason for pain. Client preference is pain so, we have to investigate the cause of pain. So Option no 'a' is not correct.

We don't need client allergy history for investigation of cause for pain so, Option "b" is incorrect.

The first preference should be to determine baseline pain and then a nurse should plan about appropriate interventions to control pain.

Administering medication is not the first process after planing intervention because you should first try relaxation techniques to reduce pain if pain still persists then you can go for medications. So, Option "d" is incorrect.
