Dudley Wayne is a 62-year-old factory worker. Mr. Wayne makes an appointment with his primary care practitioner because he has lost 15 pounds in the last 2 months and has recently noticed blood in his urine. He denies pain on urination. During the admission assessment, Mr. Wayne states that he is a two-pack-per-day smoker and just thought he was losing weight because of increased work stress. He also states that he worked 30 years in a building that was recently closed due to asbestosis contamination. Upon physical examination, Mr. Wayne complains of pain when the provider percusses and palpates his flank area. A CT scan is ordered along with an ultrasonogram; both reveal a mass in the renal pelvis. (Learning Objective 3)
A What risk factors for renal cancer does Mr. Wayne demonstrate?
A. Risk factors:
Habit of smoking (Two pack per day)
Have a history of exposed to asbestos (Chemicals) for 30 years
And also aging (62 years) and male gender may higher the risk of gets the renal cancer.
B. The symptoms of renal cancer are hematuria, weight loss, fatigue, fever, oedema over legs and ankles, pain over flank region, anorexia, dysuria
Mr. Wayne symptoms which is correlated with renal cancer are hematuria (blood in urine), dysuria (painful urination), weight loss, pain over flank region.
C. There is metastatic renal carcinoma is diagnosed for Mr. Wayne, which may not be resectable. As a palliative treatment Renal artery embolization ( A procedure to deducting or completely terminating the blood flow) may done for the management of massive hemorrhage.
D. The signs and symptoms of post infarction syndrome are
Fever
Nausea and vomiting
Flank pain
Increased white blood cell count
hematoma
The nurse should correlate the above symptoms with patient for diagnosing post infarction syndrome.
Management
Symptomatic treatment
Antipyretics for fever
Antiemetics for nausea and vomiting
Analgesics and anti inflammatory drugs for flank pain and hematoma
Cold compress may use for hematoma
After some days the symptoms will subsided spontaneously.
