NURSING PROCESS TO LIFE SPAN ISSUES TO INSURE SAFE DRUG THERAPY :
LIFE SPAN ISSUES are defined as a study of health and well being in overall life span. Before birth to up to adulthood a lot of health issues affect children like childhood diseases, eating disorders, accidents, wounds. And also includes all the ages like teenage, pregnant mothers, geriatric people, and more.
ASSESSMENT:
*Assess the health status of all the age groups, especially children, pregnant, and old age people.
*Collect detailed history of present, past medical and surgical history,
*Assess detail family history, genetic abnormalities, chromosomal abnormalities, etc
*Assess for skin changes in elderly and growth and development in children.
*Assess nutritional status of all age groups.
*Collect information on ongoing or past medical history.
*Assess for signs of infections, and so on
DIAGNOSIS :
1. Anxiety related to age related problem as evidenced by restlessness and hyperactive
2. Altered nutritional status related to vomiting as evidenced by decreased weight in pregnant mother
3. Altered thermoregulation related to infection as evidenced by increased body temperature in children.
GOAL :
1. To reduce anxiety and minimize restlessness in old age people
2. To improve body weight in pregnant mother
3. To decrease fever for children
PLANNING :
1. Assess the level of anxiety
*Provide calm and quite environment
*Maintain good communication with family members
*Monitor vital signs
*Provide recreational therapy
2. Improve body weight
*Monitor gestational age of mother
*Check for hemoglobin level in blood
*Provide diet rich in high protein, iron, calorie diet
*Check weight on regular basis
*Provide small frequent diet in between of meals and dinner
3. Reduce fever
*Monitor vital signs
*Place in a comfortable position
*Administer medications prescribed by physician
*Provide cold compress/ cold sponge according to elevated temperature in the body
*Encourage to drink cool drinks/ fluids
*Provide soft and easy digestible diet
Hence, by following of all the age group considerations to life span it is easy to focus on the age groups which need special attention and care of well being.
