LOINC - Logical Observation Identifiers Names and Codes . It is a database and universal standard for identifying medical laboratory observations. Example - 5792 -7 is used to detect glucose in urine .

SNOMED- Systematized Nomenclature of Medicine -- Clinical Terms . It is a standardized clinical terminology that is used by physicians and other health care providers for the electronic exchange of health information.

ICD - International Classification of Diseases. Latest revised version is tenth . It is used by physicians and other healthcare providers to classify and code all diagnoses, symptoms and procedures . Example is 121.9 for acute myocardial infarction unspecified.

These codes are used in EHR by the physicians and other health care workers to easily exchange patient health information , to upade health information without creating confusion ,wasting time on long writing statement by simply writing a code for a procedures, daignosis etc .
