1.brain and spinal cord are two most important part of our nervous system.with aging natural physiological changes happen in the body.mainly brain helps us for maintaining balance for movement,sensitization and helps us to perform a task.with age breakdown of nerves happen,action of neurotransmitters are decreased for this transmission slowed down and reflexes are slowed.thought process also diminishes.memory loss is the common disability that a older person can face which can lead to dementia.

2.a nurse in a acute care facility gives care for a acute stroke patient.

For aphasia,the nursing diagnosis will be-impaired varbal communication related to disease condition. So here for nursing care when communicating the with the client the nurse should keep the background calm and quiet,reduce external triggers in the environment it will help the client to communicate easily.she should give proper time to communicate.she can use drawing,symbols for communication.

For dysphagia,nursing diagnosis-risk for impaired swallowing related to disease condition.here the nurse should teach the client as well as his family members to initiate small liquid feeds for the client.tht will help to increase Oropharyngeal swallowing.

For slurred speech,nursing diagnosis-impaired verbal communication.here the nurse should teach about different communication method and should give positive reinforcement to the client.

For left sided paralysis,nursing diagnosis-activity intolerance.here the nurse can assist the client for ambulation.

3.for a bedridden spinal cord injury patient the nurse can give symptomatic management for the client.she can do position changing for the client every 3 hours.she can give back care and back massage for the client that will increase the blood flow in client's body.

4.here the nurse can give terminal care to the patient.mainly symptomatic care and terminal care can be given here.psycological support is the most important thing here, it will help to increase the quality of life of the client.
