    Steven Tyler is a 72-year-old male that has come to the hospital complaining of a sore on his foot. Patient states that he was diagnosed with diabetes 15 years ago and takes metformin for blood glucose control. Pt states he thinks he might’ve cut his foot taking his dog for a walk last week. The foot is red and edematous. Purulent drainage is noted from the sore. The patient states he knows he supposed to check his foot daily, he just forgot the last few days. He’s now worried he will lose his foot like his brother did.

●Anxiety related to disease condition as evidenced by verbalisation

Outcome:

  • The patient will have adequate knowledge on the management of the disease
  • Will be anxious free

Intervention

  • Assess the patient's level of knowledge, to plan for health education
  • Provide psychological support, to cope up
  • Provide diversional therapy to relax the patient

● Risk for infection related to poor skin integrity as evidenced by sore in the foot secondary to diabetes

Outcome

  • To control infection and heal the wound

Intervention

  • Follow aseptic techniques to prevent infection
  • Meticulous handwashing to prevent cross infection
  • Administer antibiotics as per order to destroy the microorganism
  • Regular dressing for wound healing

●Decreased mobilization related to the wound as evidenced by the sore in the foot

Outcome

  • To improve mobilization

Intervention

  • Provide patient with comfort measures to mobilize out of the bed
  • Teach active and passive exercise of the leg to improve circulation
  • Assess for pain and administer medication to relieve pain as it can interfere with mobilization
