Answer #1

Antidiuretic hormone (ADH), also known as vasopressin, is a small peptide hormone which regulates the body’s retention of water.

ADH acts in the kidney to regulate the volume and osmolarity of the urine. Specifically, it acts in the distal convoluted tubule (DCT) and collecting ducts (CT) cells.

The increase in ADH release will mean that the permeability of the DCT and CT cells to water increases and urine output decreases. This happens because ADH increases the transcription and insertion of Aquaporin–2 channels to the apical membrane of the DCT and CT cells. This allows water to move down its concentration gradient, out of the nephron and back into the blood stream. This increases the water reabsorbed from the urine, in order to decrease plasma osmolality and increase total blood volume. Water reabsorption in this manner is a passive process.

thus option A is correct answer. Water reabsorption is the cause of secondary hypertension.

Practical Application #1 Use this case report to answer the following multiple choice questions. Hypertension, both systolic and diastolic, is a common feature of Cushing syndrome. Other clinical features are likely to attract more attention than the hypertension: obesity with “buffalo hump” and “moon face,” muscular weakness, scattered bruises, and osteoporosis. The excessive secretion of adrenocortical steroids that is responsible for the syndrome is frequently due to primary disease of the adrenals, either hyperplasia or discrete tumors. However, the hyperactivity...

(LinkS to an external site. All of the above symptoms have been getting worse. Blood pressure is 170/110, respirations 12/min and some ankle edema (non-pitting) was noted. The limbs seem relatively thin and the trunk thickened, especially between the upper shoulder blades. Diagnosis was made of Cushing disease/syndrome. How could you differentiate between the two? Elevated levels of cortisol has two effects of plasma leading to increased volume: 1) up-regulation of receptors for aldosterone and 2) an increased effectiveness of...
