* Based on the subjective and objective data patient diagnosis is schizophrenia because

   incoherent speech,loss of touch with reality,flight of ideas seen toghether in this condition.

Short term goals for patient with schizophrenia are

                * Effective control of acute psychotic symptoms like agitation,restlessness,violence

                   related to schizophrenia.

                * Make sure complete participation of patient and family in this treatment process.

                * Encourage patient participation in social activities.

Nursing interventions

              * Nurse can administer antipsychotic medications(eg; chlorpromazine,haloperidol,flupenthixol

                pimozide etc) as per physicians order to manage acute clinical manifestations of the disease.

              * ECT helps to control problems like restlessness,violence,aggression in acute schizophrenia

                 so nurse can assist the patient during the procedure.

              * Participation of family members and friends is needed for better outcomes so make sure

                 their involvement in patient care.

              * Counselling sessions during the course of treatment is useful to improve psychological

                status of the patient.

               * Effective communication and good listening skills during patient care to improve patient

                 cooperation.

                * Provide psychological support through assist in daily activities.

               *Encourage patient participation in health education and group therapy.

                * provide rehabilitation services which are needed for end phase of therapy to develop

                   social,cognitive and vocational abilities.
