* Breakthrough pain occur in patient with different types of chronic painful illness and dose of the pain

medicine become ineffective to control the severity of pain.

* Plan of care of patient with breakthrough pain includes,

                       * Reduce the pain level of the patient.

                                  * Use of transmucosal opioid medicines(eg; fentanyl) to reduce breakthrough

                                     pain level.

                                  * Use of anti inflammatory drugs (acetaminophen) to treat chronic pain before

                                     the management of breakthrough pain.

                                  * If the breakthrough pain is due to progression of chronic diseases like malignant

                                    carcinomas then take chemotherapy,radiation and surgical management to

                                    control the pain.

   * Provide comfort to patient with breakthrough pain.

                                    * Provide comfortable positions to reduce the stress level of patient.

                                    * Give psychological support to reduce the anxiety level of patient due

                                       to pain.

                                    * Participation of family members in patient care to eliminate fear of patient

                                       related to therapy.

                                    * Assist daily activities of patient if she can't able to perform due to severe pain.

                                    * Provide comfortable devices like pillows,back rest to maintain comfort level

of the patient.
