Devise a flow chart or map of the process. After the process is mapped, it becomes clear what interventions are needed. Define the intervention that will address the problem and who will be involved (whose behavior will change as a result of the quality intervention). Submit your flow chart per your professor’s instructions. Share your flow chart or map with your group members, and negotiate any differences in the causes and proposed interventions. Recruit one group member to record the problems and interventions in Section 3 of your group’s Quality Improvement Plan template.

Pesent days all are suffering with health problems due to the their unhealthy behaviours,unhealthy life styles and unhealthy eating habits.Most of the populations needs medical care according to their condition.If they get adequate care and attention and cure from their disease the health care professionals has to identify the problems of the patient accurately,then apply interventions according to their medical condition,thus offer a high quality care by forming a collaborative team by the medical professionals.

The present day medical conditions that suffers the population are:

*Heart Diseases

*Arthritis and osteoporosis

*Respiaratory diseases such as asthma,bronchitis,pneumonia,T.B etc

*Cancer

*Alzheimer's disease

*Diabetes

*Risk for falls

*obesity

*Depression etc.

These all conditions what ever we said need adequate and timely care to avoid all complications and patient can improve from their condition.

The health care system has the important role to ensure patient safety,and offer a high quality and efficient care.Thus improves the client satisfaction with out developing any complications or disabilities and health care providers has gain the confidence by seeing the health promotion of the patient.

Quality improvement is a systematic,formal approach to analyse the health care staff, regarding their practice performance and the efforts what ever they are following to improve the performance.

We can improve the quality improvement plan action by:

*The organization has to implement changes and sustain improvement and the staff who are working in the organization has to make and adapt the changes at different rates.According to adapting the changes thay can compete with the challenges then only they can go forward easily to acheive the goals.For Quality improvement all the staff in the organization have their own roles and responsibilities.The staff invoved in the quality improvement team are :

$\tiny \rightarrow$ Data entry person,who enter the data available from the work of the team,submit the reports regularly to the specified person

$\tiny \rightarrow$ Provider champion,who regularly works with the patient whose care is directly affected by QI teams

$\tiny \rightarrow$ Operations Person,The persons who were directly involved in the quality improvement team are nurses,nutritionists,social workers,pharmacists or others.

Create a collaborative team of clinicians,specialists,nurses,nutritionists,dietician,pharmacists,lab and radiology,patient counseller and other depatmental staff $\rightarrow$ Create a preplan regarding the intervention with a safe,effective and standardized manner to improve the quality of the services provided by the organization and staff $\rightarrow$ Specify their own roles according to the their specialized area and population present condition $\rightarrow$ create a positive attitude from the patient by explain the condition and proper communication thus improves the co-ordination from the patient $\rightarrow$ patient react to the quality improvement program $\rightarrow$ regular assessment of the program $\rightarrow$ the data regarding the quality improvement program should be correctly entered and submitted to the authorized department $\rightarrow$ if any improvement or changes needed apply it $\rightarrow$ review the programme success.

If the patient improved from their present condition with out developing any complications and disabilities that improves the client satisfaction due to the highly effective,safe and standardized care by the collaborative team approach.
