Scenario: Mr. Barnes is your client. He has been wandering away at night because of advancing dementia. He doesn’t wander far and usually ends up on the neighbor’s porch. You are worried about his safety and the neighbors have started to ask questions about his behavior.

Answer :

Dementia is the loss of brain or mental function such as thinking ,memory ,and reasoning .

- it will affect the persons daily functioning .

people with dementia walk seemingly ,for a variety of reasons, such as boredom ,medication side effects or looking for something or someone.

- they also may trying to fullfil the physical needs such as thirst ,hunger ,use of toilets and excercises.

- discovering the trigger for wandering are not always easy but as a health care provider we need to deal with insites of the various behavior.

nurses responsibilities for dementia clients :

• Nurse should encourage the patient for maitaining of regular daily living schedule ,so that patient will have touch with daily living activities.
• allow the freedom to move around in the safe environment wii give a sense of autonomy and relaxation.
• nurse should not restraint the client .
• nurse should keep identification tag on patients will facilitate a safe return to the ward.
• nurse should administer the drugs at appropriate time and dose ,should not leave the medication by the patient.
• nurse should provide adequate diet.
• keep the environment simple and pleasing ,remove all unwanted things around the patient bed side.
• orient the clieny for time ,place ,person and bring the patient near the window and show him the surroundings.
• place the black mat or paint a black space at patient front porch ,this may appear to be an impassible hole to the client with dementia.
• add security system .
• mask the front door of patient with signs of stop or do not enter may be help ful .

• Good communication skills will enhance the nurse to handle difficult situation or difficult behaviour of clients.
• nurse should explain the detailed health condition of the neighbors such as what is dementia ,symptoms of dementia .
• nurse should reassure the client that dementia client will not harm to neighbors .
• explain about how the people will behave with dementia and wandering behavior .
• should give confidence to neighbor that the dementia client should not disturb others.
• And advice the neighbors for do not critisize the dementia client ,it may disturb the emotional status of dementia client .
• Advice the neighbors to call for help if dementia client is doing wrong .
• nurse should inform the neighbors ,if dementia client going ear ,just tell the neighbors to call the nurse.

• as a health care provider i have explained about the whole behaviour of the dementia client .
• i hope that the neighbors are understood well about the dementia client behaviour .
• and i think they will react positively to my words such as adjusting with dementia client behaviour
• and i hope that they dont critisize the dementia clients aboit his condition .
• and i hope they will inform to me when the dementia client doing wrong or going far.

3) how do you know that you are acting ethically as mr.barnes ' behaviour :

• By maintaining good therapeutic relationship with dementia client
