1) Modifier 50 - Bilateral procedure. This codes indicate performance of surgery by a surgeon at same session but in two different regions. Here carpal tunnel surgery is performed in two wrists.

2) Modifier 76 - This is coded when procedure is repeated on a same day by same physician. In this scenario lumbar puncture is done on 1:00 and repeated again on 5:00 of same day.

3) Modifier 26 : Radiological procedures done by a technician but the report interpretation is done by a physician is reported with mod 26.

4) Modifier 59 - Physician performing evaluation and due to certain circumstances he performed a procedure to remove lesions in the arm it is considered as distinct procedure.

5) Modifier 25 : Evaluation and management service is defined as time based codes , assessment done in a physician office. The procedure performed under E/M service can be appended with modifier 25

6) Modifier 55 : this modifier helps to understand global surgic package and the physician append modifier 55 only to the service of treating post operative infections.
