The patient has a history of alcohol and cocaine dependence since age 17. During the course of admission, the patient was placed on hydrochlorothiazide 25 milligrams for hypertension, to which he responded well. He participated in this rehabilitation program and worked rigorously throughout. On discharge, the patient is alert and oriented x3. Mood is euthymic. Affect is full range. The patient denies Si, HIl, denies AH, VH. Thought process is organized. Thought content-no delusions elicited. There is no evidence of psychosis. There is no imminent risk of suicide or homicide.
