A woman presents to her doctor with a 2 month history of epigastric pain. She gets full after a few bites of food but was not concerned as she was on a diet and she was able to loose 11 lbs. in the last 2 months. She has a been under much stress due to her husband being out of work. She thought to seek care when she noticed some blood in her stool and had started to experience nausea. You decide to order tests and they come back positive for H Pylori.

1) Looking at her symptoms and the H. Pylori presence, I think it would be important to differentiate between gastric cancer, GI polyps, duodenal ulcer, gastric ulcer or gastritis. I believe the major 2 I should be looking at are cancer and ulcers.
2)To investigate between cancer and an ulcer I would order a radioimmune assay of gastrin levels, an upper GI endoscopy as well as a biopsy to rule out cancer. I would also order a blood test and stool test to see if she is anemic and really does have blood in her stool. From the imaging I would be better able to decide between cancer and an ulcer.
3)Other systems that could be infected by H pylori could be the stomach or duodenum. If this is cancer it can metastasize to other systems of the body.
4) If there is evidence of other systems showing a similar pathology I would do a barium meal study. I could also do another scope.
