Answer: Hyperglycemia is a condition when there is an increase in the blood sugar level. Nursing diagnoses includes the monitoring of the blood glucose level of the patient. This will keep constant maintenance of the glucose levels. The fasting and non-fasting range should be determined i.e. fasting glucose range of less than 100 mg/dL is normal. The nurse should check the other symptoms of diabetes such as polyuria, weight loss or blurred vision etc.

Nurse should educate the patient that they need to monitor their blood glucose level. Nurse should inform the patient about the healthy diet and diet control such as avoiding refined sugar, caffeinated drinks. These teachings may prevent hyperglycemic conditions. Indulgence of physical activities is most important as it willl slow down the risk rate.
