How might a decrease in physician income, possibly as a result of an increase in the number of physicians, affect the physicians role as the patients agent ?

If the physicians income decrease then they might suggest such services which will increase the cost of treatment. That is they will induce demand by increasing demand for unnecessary services, which are not required.  They will act as an agent to those services and will charge commission on it.

Thus, they will create more need for the medical care in the eyes of their clients. Hence this will increase their income.

This will make them an agent.

(Although it is an unethical practice but in order to improve their income they will induce the demand for medical services.)
