Four-year-old Owen Russiano has been brought by his babysitter for an emergency appoint- ment because of severe nausea and vomiting. Because he is so small, Dr. Miller is concerned that he may be dehydrated. Dr. Miller has ordered medication to be administered stat, via a rectal suppository. Samra is concerned because the boy seems very fearful, and she is worried she will not be able to administer the medication because of the child's behavior.

1.how should Samra proceed to fillow Dr. millers instructions. 2. what should Samra do to help her administer the medication to the patient. 3. Owens babysitter has brought him in for the office visit, will Samra need to obtain parental permission before administering the medication.

1.Samra should proceed in the following way to avoid consequences

• Gathering information in regards to the procedure from a senior or from a colleague who has experience in the particular procedure. In case if it is not possible it is always best to inform the physician that they are not experienced with the procedure and needs guidance to do it in the right way in a humble way

2.Samra should do the following to administer the medication

• Creating a friendly rapport with the child
• The baby sitter can be along with the baby to reduce fear, though the chance are minimal without the real parents who can comfort their child to the best than others.
• Explaining the procedure to the baby sitter after following the protocols of the hospital

3.A child with four year old cannot make a decision on medical treatment. Here parents acts as the first person to provide consent in regards to any procedure. If the baby sitter was allowed to act as a guardian in regards to medical treatment appointed by parents then it can be considered. Otherwise to avoid legal consequences and in the best interest of the child parental permission is absolutely necessary before administration of any medication.
