Ans:- Evaluation and management is a medical coding process that supports medical billing.
E/M coding is the process by which physician- patient encounters are translated into 5 digit CPT code to facilitate billing.
CPT stands for "current procedural terminology".
These are the numeric codes which are submitted to ensures for payment.
Every billable producer has its own individual CPT code.
There are different types of E/M codes for different types of encounters such as office visit or hospital visit.
Within each type of encounter their is different type of levels of care.
For example:- for new patient office or other outpatient visits CPT code is 99201-99205,
For established patient office or other outpatient visits CPT code is 99211-99215.
Stpes of process for choosing E/M codes are as follows:- a) Identify the category and subcategory of the service.
b) Review specific notes and instructions for the selected category and subcategory.
c) Review the narrative descriptors within the category and subcategory of E/M services.
d)Using the definition provided under each level of service, determine the extent of history obtained.
e) Determine the extent of examination performed using the definitions provided under each level of service.
f)Determine the complexity of medical decision making.
g)Select and verify the appropriate level of E/M service.
