1. The term multidisciplinary means combing and involving several academic disciplines and professional specialisation to deal with a problem .

The three hospital committee are :-

- Ethics committee meets to discuss ethical issues and problems

- Joint conference committee acts as a liaison between the governing body and administration

- surgical committee is the committee which conducts a thorough check up and assessment of the patient before surgery and determine the need for surgery

2. Diagnosis index is the form which have tabulation including Patients name ,sex ,age , hospital ID , diagnosis and other conditions present . It is used to avoid confusion about diagnosis among the patients

Procedure index is a index which contains all the name of the procedures either minor or major .
