ANS)1) One type of bias in the observational study is INFORMATIONAL BIAS.

Usually the Informational bias arise from incorrect documenting of individual aspects and that may be risk component or may be the illness.Accompanied by ongoing variables like the BP( Blood Pressure),this is usually known as the measurement error and accompanied by the unqualified variables like the tumor phase,this is usually called as the misclassification.

One example or the error is,

The examiner is asking question to the patient like,From which age onward you have chickenpox? How much quantity of fruits did you consumed before 2 weeks? we can say that the reply to this questions will be absolutely indefinite.

We can say that both the Misclassification and Measurement error comes form absence of care by examiner or due to the low standard of measuring device.But anyway they are mostly made by the mistakes in manner or period of categorization.But we can solve these complications partly by perfect planning,but not every time be rectified by statistical assessment.
