Answer #1

1. If a male patient denies that he smokes, but his fingers are stained yellow from nicotine and he smells of tobacco smoke, the questions to be asked to clarify are:

  • Do you use any other form of tobacco?
  • What is that yellow stain on your fingers?
  • You do have a strong tobacco smell, do you have any reason for that if you don't smoke?
  • Are you willing to do a tobacco smoker's test?

2. a) The health and wellbeing of family members contribute to the family history part in a patient's medical record. When the family history of a patient is unknown as in the case of an adopted patient, in the medical record this will be given either as unknown, patient adopted or as biological parents not known.
