NUR 111 NUTRITION CASE STUDY Rita Tojino is an 87-year-old woman who has lived in a skilled nursing facility for the last 6 months. She has a past history of CVA and could no longer live on her own. She has become forgetful over the past 2 years She has limited verbal ability, and can use her hands with one- on-one coaching. She is brought to the dining room along with all the other residents who need assistance with feeding. She usually sits in a geri-chair which puts her in a position not conducive to eating. The dining area is very noisy with the television turned up very loud She has lost 8 pounds since admission and her current BMI is 17.3 She had a complete upper denture but it disappeared. Presumably it was wrapped in a tissue and was inadvertently thrown in the garbage The RN checked Ritas admission assessment in her chart to ascertain her favorite foods, but it was never filled out. One of the other nurses suggests a feeding tube, but Ritas nurse knows there are other steps to take before a feeding tube is inserted. Open With Print

Care plan for nutrition include the following :-

- Plenty of fluids

- Provide small frequent feeds

- Advice patient to not sleep or lie straight after intake of food ,always have the bed 30degree elevated.

- use of additional eating supplements to fulfill the requirements of certain components.

- use of soft diet and liquid diet more

- diet should provide all the vital components to keep the patient healthy.

- if the patient status is nill per mouth ,then NG tube has to be inserted to provide meals and if the NG tube is impossible to put then IV access has to be utilized to give TPN .
