She She is brought usually sits in a geri-chair which puts her in a position not conducive to eating. The dining area very noisy with the television turned up very loud. She has lost 8 pounds since admission and her current 8MI is 1 She had a complete upper denture but it disappeared. Presumably it was wrapped in a tisue and was inadvertently thrown in the garbage. The RN checked Ritas admission assessment in her chart to ascertain her favorite foods, but it was never filled out One of the other nurses suggests a feeding tube, but Ritas nurse knows there are other steps to take before a feeding tube is inserted * stale Dagnosis stat EiooieS

1. Diagnosis is Disability along with malnutrition

Disability is any continuing condition that restricts the everyday activities.

Malnutrition is the lack of sufficient nutrients in the body

2. The problems are :-

- she cannot take care of herself due to increase in age and disability

- problem of forgetting things

- limited verbal ability

- complete upper denture is absent

- difficulty to maintain a proper position for eating.

3. The cause of all these problems is:-

- increase in age

- decreased intake of nutrients in the diet

4. Sign and symptoms are:-

- BMI of 17.3

- inability to do activity of daily living

- forgetting things

- inability to talk clearly

- difficulty to maintain a posture

- can't sustain loud voices
