  • Dressing dry and intact
  • Foley catheter draining clear yellow urine of 60–70 ml/hour
  • Positive movement and sensation in the legs
  • Epidural site clean and dry
  • Alert and oriented and denies pain at present
  • Attempted breast-feeding in the recovery room, but the infant was sleepy.

According to the the given scenario

Question 1) physical assessment is most important thing to perform both of the mother and newborn . Check for vitals of mother and newborn . Assessment of emotion of mother is also very important.

While checking vitals special concern to be given on respiration of new born.

Avoid any exposure to the body fluids.

Since the couple has not attended any training in pregnancy she must be aware of skin to skin contact of mother and newborn. Exclusive breastfeeding , how to breastfeed and potential risks .every finding should be reported to the mother also.

2) According to the peak model of readiness to question to be asked is

Ask whether the mother is physically stable or not ?

Is mother is comfortable with the settings or feeling distracted?

Whether the mother is emotionally stable or not?

Whether the mother is feeling motivated and oriented or not?

How much the mother know about the topic or what is her knowledge base?

What learning method she is comfortable with?

3) Major points should be covered before discharge when teaching the mother are

Is mother and child is comfortable or not? Look for their vitals .look for any bleeding.

Ask the patient to whether she learned about the everything or not

Ask the patient to demonstrate about the method in her own words.
