***QUESTION IS IN BOLD: Rita, a 70 year old female, comes to the clinic complaining of chronic cough, drenching night sweats, fatigue and persistent fever, and weight loss. Three weeks ago she was seen in the ER and given azithromycin for pneumonia but has not noticed any improvement in her symptoms. She says she has shortness of breath at rest, decreased breath sounds on right and a coarse cough. Her lab results are as follows:

HGB 14.0, WBC 6.8, Normal Neutrophils, lymphocytes, Sodium 144, Potassium 3.6, Chloride 110, Bicarbonate 24, Creatinine 0.8, BUN 24, ALT 20, AST 40, TB Quantiferon - negative, Sedimentation rate 78. CT scan shows Ipsilateral Hilar Adenopathy with enlarged right hilar subcarinal and paraesophageal lymph nodes.

Using this information, what lab tests should you run and what would you be looking for in each test?

The CT scan results interpret that the patient may have a small cell carcinoma.So the following lab test may be performed

  • PET scan to determine the extent of the spread
  • Endobronchial ultrasound to exactly locate the site
  • Bronchoscopy to locate exactly the site
  • Biopsy a confirmatory test.
  • Sputum cytology to rule out the oresrncr of cancer cells in it
