1.ASSESSMENT:

Dyspnea

NURSING DIAGONSIS:

Impaired gaseous exchange related to immature functions of pulmonary system.

NURSING GOAL:

After evaluation and monitoring with interventions the neonate will be weaned to room air without any supplementary post 24 hours.

NURSING INTERVENTION:

  • Assess the respiratory status and note any form of respiratory distress eg) Nasal flaring, Gurnting, Tachypnea, Rhonchi or crackles
  • Assess for any signs of cyanosis.
  • Monitor the laboratory studies.

RATIONALE:

  • Tachypnea indicates signs of respiratory distress when the >75cpm/min within first 5 hours of life.
  • Lack of oxygen results in cyanosis.
  • Prevent the future complication.

OUTCOME:

After 24 hours of intervention the patient will be able to breath without oxygen supplement.

2.ASSESSMENT:

Nutritional status

NURSING DIAGONSIS:

Altered nutrition less than the body requirement related to prematurity as evidenced by weak feeding reflexes.

NURSING GOAL:

After evaluation and monitoring the neonate will be able to gain weight upto 30g/day with persistent growth.

NURSING INTERVENSION:

  • ​​​Assess the neonate reflexes eg) swallowing, sucking, coughing.
  • Instill breast milk/Formula milk over 20 mins with 20ml/20min.
  • Position Right side with Head elevated 30 degree.

RATIONALE:

  • ​​​​​​To know the appropriate feeding method of the infant.
  • To prevent risk of regurgitation
  • Facilitate gastric emptying and prevent reflux.

OUTCOMES:

Bring out good sucking & swallow reflexes and add 10g to body weight.
