Physiological nursing diagnosis are for this patient are :-

Nursing diagnosis - Shortness of breath related to disease condition COPD and CHF evidenced by usage of strenuous muscles.

Nursing interventions :-

- Assess the vitals of the patient

- Provide the patient semi Fowler's position

- Administer oxygen by mask or nasal cannula as per doctors prescription .

- Teach patient deep breathing exercise , coughing exercises

- provide a calm environment for the patient

- provide patient psychological support and relieve anxiety related to shortness of breath

Nursing diagnosis - Chronic back pain related to disease condition cancer of kid ey evidenced by facial expression

Nursing interventions :-

- assess the vitals of the patient

- assess the pain using pain scale

- administer analgesics as per physicians instructions

- use non pharmacological methods like distraction , music all therapy etc

Nursing diagnosis - Edema related to hypertension and congestive heart failure.

Nursing interventions :-

- assess the vitals of the patient

- assess the grade of edema

- administer antidiuretics , antihypertensives and medications for congestive heart failure as per the doctors instructions

- monitor weight of the patient daily as it is the best method to evaluate the edema .

- administer IV fluids and oral fluids with appropriate measure as excess might adverse the edema
