Answer :

Nursing diagnosis 1 :

Ineffective breathing pattern related to airway abstraction as evidenced by short ness of breath.

Rationale to choose nursing diagnosis is patient is having COPD and CHF and short ness of breath it is the most priority problem to treat first.

Goal : To maintain effective breathing pattern.

Interventions :

* Assess the breathing pattern as client whether it is regular or not,

* Assess the oxygen saturation levels on the monitor by using pulse oxymeter.

* provide comfortable position that is Fowlers position with the support of backrest or pillows.

* administer oxygen by using nasal prongs 2 lit per minute.

* provide mobilization, to loosen the secretions and and clear the airway can improve normal breathing pattern.

* Administer bronchodilators to dilate the bronchus and improve breathing pattern.

Evaluation :

After implementation of these nursing interventions client may get reduced from shortness of breath and improves oxygen saturation level from 80 % to 95 %.

Nursing diagnosis 2 :

Fluid overload or fluid volume excess related to inadequate excretion of fluid due to Chf and CA kidney as evidenced by putting edema on legs.

Goal :

To maintain normal fluid volume and reduce excess fluids from the body.

Interventions :

* To assess the edema area whether it is pitting or not.

* advise patient to take salt restricted diet.

* monitor weight daily to know the fluid volume reduction.

* administer diuretics like furosimide to excreat excess fluid volume and also increases the normal breathing pattern.

* restricted fluids should be provided to the client.

Evaluation :

After implementation of nursing interventions the client may be reduced from edema and fluid overload may be decreased.
