The 5 phases of Nursing process are

  • Assessment:This includes collection of  patient information by observation and from patient conversation. This is called as
    • subjective data where the patient describes their needs and problems.for example a patient with pain in abdomen with pain in abdomen and fever
      • Feeling nauseated
      • Having pain in abdomen
      • Feeling cold
    • The objective data is done by the healthcare professionals by observation
      • Temperature:101
      • Facial grimace due to pain
      • Pain on palpation
  • Diagnosis:The complaints of the patient are formatted as a nursing diagnosis
  • Goal/outcomes:the planning is done to relieve the patient from the problem
  • Intervention and rationale:the planned work is implemented here describing the rationale in doing the work
  • Evaluation:the patient is assessed for the outcome

a.Fluid and electrolyte

  1. Imbalanced electrolyte level related to renal dysfunction as evidenced by increased sodium level ,creatinine ,confusion ,disorientation
  2. Risk for fluid volume deficit related to dehydration as evidenced by vomiting ,diarrhea,dry skin
  3. Altered sensorium related to electrolyte imbalances as evidenced by confusion

b.Mobility

  1. Impaired mobility related to loss of bone strength as evidenced by fracture
  2. Risk for fall related to oldage as evidenced by loss of hair secondary to Parkinson's disease
  3. Risk for pressure ulcer related to immobility as evidenced by bed ridden status

c.Stress and coping

  1. Ineffective coping mechanism related to neurological disturbances as evidenced by feeling depressed, isolated
  2. Risk for self harm or injury related to stress as evidenced by showing anger
  3. Anxiety related to disease condition as evidenced by verbalisation
