1.Nursing process is systematic, scientific method of providing client centered care. It has 5 phases:

  1. Assessment: it involves collection of data relevant to patient from patient directly or from caregivers.
  2. Nursing diagnosis: it is clinical judgement based on data collected to plan treatment for the patient.
  3. Planning: it involves formulation of goals and outcomes for the patient.
  4. implementation: it involves application of planned interventions on patient.
  5. evaluation: it involves reassessment to find out if planned goals are achieved or not.

2. subjective data is type of information that can not be measured. It relies on verbal statement of patient only.for eg patient complaint of dizziness, itching and shortness of breath

3. objective data: it relates to the data that is measurable . For example height, vital signs, intracranial pressure

4. nursing diagnosis relate to fluid and electrolyte

fluid volume deficit, fluid volume excess, hypokalemia

nursing diagnosis related to mobility: impaired mobility, risk for pressure ulcers related to immobility, apathy related to immobility

nursing diagnosis related to stress and coping: ineffective coping, risk for self directed violence, risk for suicide
