It also provides care coordination for patients and families, that provide better health outcomes. It is a system focusing service for the patient with coordination, sharing medical information, goals, responsibility for the patient outcome. it is a multi-specialty group practice.
Example: A patient who undergoes a mastectomy surgery it is must be treated by a system that includes primary care physician, surgeon, oncologist, technician, nurse and rehabilitation therapist for lifestyle modifications.It benefit physician for cost-effective administration, financial support, patient satisfaction, ownership potential, and high market place.
Barriers to the integration of care in health care settings lack of collaboration that provide an unsuccessful implementation of care. There is a barrier in promoting awareness and taking action, making innovation, implementation, lack of resources, technology interoperability of IT system, lack of funding, high cost, lack of physician, high malpractice practice, lack of knowledge for quality health care services. This can be corrected with managers, health care professionals, policymakers to overcome the barriers to integrated health care in health care settings.
