She is not anticipated to be able to be weaned from the ventilator, and the physicians have asked the patient’s family for permission to create a tracheostomy. The family is concerned that the patient will not be able to speak again. What is your response? What are some possible concerns for patient care in the immediate postoperative period? Tracheotomy : it means opening of trachea with surgical procedure to maintain respiration by using mechanical ventilation.

* the family is concerning that patient will not be speak again, for that my response is after tracheotomy we can maintain the adequate ventilation to the patient, so that the recovery will be taken place,

* after that if we will close the trachea withe sterile gauze pad, the may speaks again,

* there is no problem with tracheotomy, during the time of opening the patient can not talk but after closing it again she can able to talk like that I will educate the patient family members.

Possible concerns for the immediate postoperative care :

* we need to observe for bleeding,

* abserve for Pneumothorax,

* provide comfortable position,

* check the vital signs and oxygen saturation levels,

* see for secretions and do the suction with sterile techniques.

* support with mechanical ventilation.

To prevent tracheal damage :

* we need to do suction in gentle manner with needed and adequate pressure.

* provide comfortable position,

* use sterile and non infective material to fix for the trachea.

The tracheotomy is not permanent,

If the patient is recovers after some days, we can check her saturation and respiration levels with closing of trachea by temporarily,

If she is maintaining good saturation levels and ventilation with out mechanical support means we close the trachea.

Here there is no image damage after tracheal closure. It can be cured.
