Fibromyalgia is a chronic disorder characterized by widespread,nonarticular muscular skeletal pain and fatigue with multiple tender points.People with FMS may experience non-restorative sleep,morning sickness,irritable bowel syndrome and anxiety.The former name for this disorder is ,fibrositis,implied inflammation of the muscles,and soft tissues.

Research continue to focus on identifying the underlying causes and pathophysiologic mechanism of FMS..There is general agreement that FMS is a disorder of central processing with neuroendocrine/neurotransmitter dysregulation.The pain amplification experience by the affected patient is due to abnormal sensory process ing in the central nervous system.Numerous studies now identify multiple physiologic abnormalties in the FMS patient,including increased level of substance P in the spinal cord,low levels of blood flow to the thalamus ,dysfunction of the hypothalamic -pituitary -adrenal axis,low levels of serotonin and tryptophan and abnormalities in cytokine function.Serotonin and substance P play a role in mood regulation,sleep,and pain perception .Changes in the HPA axis can also negatively affect a persons physical and mental health.,leading to an increased incidence of depression and a decreased response to stress.

The treatment of FMS is symptomatic and require a high level of patient motivation.The nurse can play key role in teaching the patient to be an active participant in the therapeutic regimen.Analgesics such as Acetaminophen are effective for some patients.Stress,fatigue and sleep disturbances can be helped by taking a low-dose tricyclic antidepressant such as Amitryptilline .SSRIs are prescribed at low doses during the day and someties combined with a tricyclic antidepressant at bedtime..The skeletal muscle relaxant Flexeril is also commonly used to treat sleep disturbances.

Dieticians often urge FMS patients to limit their consumption of sugar,caffeine,and alcohol because these substances have been shown to be muscle irritants.Vitamin and mineral supplements may be appropriate to combat stress,correct deficiencies,and support the immune system.However unproven miracle diets or supplements should be carefully investigated by the patient with FMS and discussed with the health care provider before using them.
