b.Some of the ways to explain that something is wrong are explaining the status of the patient in relation to the surgery and its prognosis. By using the brainstorming approach to find varied solutions, morphological analysis, lateral thinking in advance can be used as approaches for problem solving.

c.Some of the steps which could have been taken in regards to inking are
