Option D

Since in basic medium, we should add H2O molecules at oxygen rich side and double the no of OH- on opposite side.

Here 2 Oxygen atoms are more at Reactants side, hence 2 H2O added at reactant side and 4 OH- ions on opposite side, gives balanced equation
