In order to guide this patient for smoking cessation following nursing intervention and plan can be led-

DIAGNOSIS-

Knowledge deficit and ineffective health deficit evidenced by patient smoking problem.

PLANNING-

The nurse will discuss smoking cessation benefits with the patient, will talk about tbe smoking cessation aids and refer to the aids.

Rationale- the motivational interviewing is important for providing better care and cessation of smoking to reduce further damage to lungs.

IMPLEMENTATION-

Weekly sessions of motivational interviewing use of nicotine pacthes and other aids and referral to other facilities for cessation.
