In countries where nutritional status and environmental sanitation have improved ,the incidence of vector -borne diseases such as yellow fever and Japanese encephalitis fecal oral infections such as cholera and typhoid fever and some person to person trandmitted infections sich add tuberculosis have declined. The need for developing protective agents or vaccines against these diseases has diminished in such countries. In the other hand, the common childhood infections such sdeadles, mumos etc cannot be controlled except through prophylactic immunization. Countries in which economic and social development are relatively slow have to drpend on vaccines against these diseases in order to reduce their incidence

Before approving 'newer' vaccines by experts of a country or of the WHO, the efficacy and the absence or low frequency of adverse are carefully evaluated. Some of the older vaccines we're used eidely before such careful scrutiny was possible. The collective experience or recent evaluation hsd shown that dome of them are inferior to the newest vaccines in terms if safety or efficacy. Sound information and discretion are necessary before vaccines are chosen for routine use in children.

In paediatric routine immunization is given to individual children for their personal protection In public health practice individual as well as community protection is dedesired. Ideally a susceptible children should be imunized. Therefore the opportunity if s ick visit should also be uyilized to promote immunizstion Minir illness such as URI are not contraindications for administing sny vaccine. The practice if setting apart one or two days each week for immunization often leads to many missed opportunities to protect them needy children

In order to control vaccine-preventsble ,WzhO and other member countries have established the aExpandedthe Expanded program. It is the dutyof every paediatrician to support this national effort.
