In the medical related conditions it is very important that we get all the informations and adequate informations related to the patients condition and that we should not handle things hastily.

We should always be specific about the patient and as it is related to the people's health we should never settle for the less. Hence, I would not settle with other things rather I would try to talk to the assistant of the things that has been missing and try to explain the situation to him. If the assistant will not be able to recall the informations than it is important to talk to the physician himself to gather the remaining informations for a providing proper care and treatment to the patient.
