Documentation of dangerous drug is very important doctor should prescribe the drug name , dose and route clearly in the prescription also he should mention patient name , age , room number, and diagnosis of the patient and there should be signature and name of the doctor who ordered medicine. Before admininistering the medication staff should double check the doctors order inorder to avoid medication error . Need to document in the nurses note the time , route, dose and name of the drug given .
