1. An inotropeis an agent that change the force or energy of muscular contractions of cardiac muscles.

Negatively inotropic agents reduce the force of muscular contractions.

Positively inotropic agents increases the strength of muscular contraction.

One of the most important factors affecting inotropic state is the level of calcium the cytoplasm of the muscle cell. Positive inotropes usually increase this level, while negative inotropes decrease it. However, not all positive and negative drugs affect calcium release.

Indications of ionotropic agents are:-

decompensated congestive heart failure, cardiogenic shock, septic shock, myocardial infarction, cardiomyopathy, etc.

To reduce morbidity and mortality in congestive heart failure.

Amiodarone is the drug of choice for the treatment of VT associated with MI.

3. NARCAN (naloxone) is indicated for the

• complete or partial reversal of opioid depression, including respiratory depression,
• induced by natural and synthetic opioids, including propoxyphene,
• methadone and certain mixed agonist-antagonist analgesics: nalbuphine, pentazocine, butorphanol, and cyclazocine.

For the initial reversal of respiratory depression, naloxone hydrochloride should beinjected in increments of 0.1 to 0.2 mg intravenously at two to three minute intervals to the desired degree of reversal, i.e., adequate ventilation and alertness without significant pain or discomfort.
